Click on the links below and print out the forms that you need.  Forms can be turned in at the Due West UMC Children’s Ministry office in Admin 2 or in the Children’s Check-In area just inside the “sunflower doors”.  They can also be mailed (3956 Due West Road, Marietta, GA 30064). There is no online registration.

    Adult volunteer form
    High School Youth volunteer - 9th – 12th (2012-2013 school year)
    Middle School Youth volunteer – 6th – 8th (2012-2013 school year)

    Nursery Registration Form
    Preschool Registration Form (age 4 or 5 on Sept. 1, 2013)
    Kindergarten-4th  Grade Registration Form (2012-2013 school year)
    5th Grade Registration Form (2012-2013 school year)

We need over 100 crew leaders to effectively operate our VBS.  Adults who have not already contacted Kyle Moore (Director of Children’s Ministries) about volunteering will be assigned to crew leader slots.  If you feel that your gifts can best be used in another area, you need to Email Kyle Moore.

Training is required for all of our crew leaders.  See the Volunteer Information Sheet for training dates and other important information.
